Summary

Phacolytic glaucoma involves lens proteins triggering inflammation. Macrophages involved in glaucoma may also contribute to uveitis in the fellow eye, suggesting a broader role in lens-related eye conditions.

Background:

  • The exact mechanisms behind lens-induced uveitis and phacolytic glaucoma remain unclear.

Observation:

  • A case study of a 62-year-old female with bilateral lens dislocation into the vitreous.
  • The patient presented with phacolytic glaucoma in the left eye and mild granulomatous uveitis in the right eye.

Findings:

  • Ultrastructural analysis revealed lens fragments and macrophages containing lens proteins in aqueous and vitreous samples.
  • Immunocytochemistry confirmed foamy macrophages expressing CD68 and HLA-DR.
  • The fellow eye exhibited chronic granulomatous uveitis with mutton-fat keratic precipitates.
