Background:

Fourth branchial pouch anomalies are infrequently documented.
Understanding branchial pouch development is crucial for diagnosing congenital abnormalities.

Observation:

A rare case of an isolated branchial cyst was identified.
The cyst contained ectopic thyroid tissue, an unusual finding.

Findings:

This case, along with prior reports, strengthens the hypothesis of fourth branchial pouch contributions to thyroid development.
Ectopic thyroid tissue within branchial anomalies suggests a shared developmental origin.

Implications:

These findings may refine our understanding of thyroid embryogenesis.
