Summary

Children with asthma generally have normal immunoglobulin G (IgG) subclass concentrations. A slight increase in low IgG2 levels was observed, but without clinical significance. Severe asthma showed higher IgG4 levels.

Background:

  • The relationship between immunoglobulin G (IgG) subclass deficiency and asthma is debated.
  • Previous studies may be confounded by systemic steroid use, which can cause hypogammaglobulinaemia.

Purpose of the Study:

  • To investigate IgG subclass concentrations in children with asthma who were not on systemic steroids.
  • To compare IgG subclass levels between asthmatic children and healthy controls.

Main Methods:

  • Measured IgG subclass concentrations in 200 children with asthma (aged 2-17) and 226 healthy controls.
  • Asthma severity was assessed using the Asthma Severity Score (ASS).
  • Participants had not received systemic steroids for at least six weeks prior to the study.

Main Results:

  • Mean IgG subclass concentrations in asthmatic children were within the normal range (1 SD) of controls.
  • A trend towards higher IgG1 and IgG4 levels was noted in asthmatic children.
  • A slightly higher prevalence of low IgG2 concentrations (< 2 SD) was observed in children with asthma (4.5% vs 2.2%).
  • Children with severe asthma (ASS 3-4) had significantly higher IgG4 levels compared to those with mild asthma (ASS 1).
  • No significant differences in IgG subclass concentrations were found between atopic and non-atopic asthma.

Conclusions:

  • In unselected children with asthma not on systemic steroids, mean IgG subclass concentrations do not significantly differ from healthy controls.
  • Low IgG2 concentrations were slightly more common but not clinically distinguishable.
  • Elevated IgG4 levels may be associated with severe asthma.
