Summary

Preeclampsia risk in first pregnancies may be linked to mothers sharing too many paternal human leukocyte antigens (HLA). This genetic sharing was significantly higher in women with preeclampsia compared to healthy mothers.

Background:

  • Preeclampsia (PE) is a serious pregnancy complication.
  • The role of human leukocyte antigens (HLA) in PE development is not fully understood.

Purpose of the Study:

  • To investigate the association between maternal-fetal HLA antigen sharing and preeclampsia.
  • To explore potential genetic factors contributing to PE in primigravidae.

Main Methods:

  • Study included 48 women with PE and their partners.
  • HLA antigen typing was performed on mothers, fathers, and in some cases, children.
  • Statistical analysis compared HLA sharing patterns between PE and control groups.

Main Results:

  • Children of primigravidae with PE shared at least 2 paternal HLA antigens with their mothers in 53% of cases, versus 4% in controls (p < 0.001).
  • No significant sharing was observed in multigravidae with PE.
  • Increased HLA-B locus homozygosity was noted in children of primigravidae with PE (40% vs. 4%, p = 0.01).

Conclusions:

  • Maternal-fetal HLA antigen sharing, particularly paternal antigen sharing, may be implicated in the development of preeclampsia in first pregnancies.
  • Further research into HLA genetics in preeclampsia is warranted.
