Background:

Invasive Group B Streptococcus (GBS) infections are increasingly reported in adults, particularly those with underlying health conditions.
Ocular GBS infections are uncommon, with metastatic endophthalmitis rarely documented and typically linked to endocarditis.

Observation:

Two adult patients presented with metastatic endophthalmitis due to Group B Streptococcus.
Neither patient had a diagnosis of endocarditis, a previously established association.

Findings:

This presentation demonstrates that Group B Streptococcus can cause metastatic endophthalmitis in adults without endocarditis.
The cases underscore the evolving and diverse clinical presentations of invasive GBS infections.
