Traumatic atlantoaxial distractive instability: case report

Background:

Atlantoaxial distractive instability is a rare but serious spinal injury.
Traumatic injuries to the upper cervical spine can lead to significant neurological deficits.

Observation:

A respirator-dependent quadriplegic patient presented with symptoms of atlantoaxial distractive instability.
Initial radiographic evaluation revealed an anterior atlantoaxial dislocation.

Findings:

The patient was treated with halo traction for initial stabilization.
After 8 weeks, occipitocervical fusion was achieved with positive clinical results.

Implications:

This case highlights the importance of recognizing and managing traumatic atlantoaxial instability.
