Summary

Occupational asthma was diagnosed in a woman exposed to heated polypropylene. Heated polypropylene should be recognized as a cause of occupational asthma.

Background:

  • A 35-year-old nonatopic woman presented with symptoms suggestive of occupational asthma.
  • She worked as an operator in a plant producing polypropylene bags for four years.

Observation:

  • The patient experienced a productive cough, dyspnea, wheezing, and rhinitis for three years.
  • Spirometry showed significant airflow obstruction (FEV1 43% predicted) while at work.
  • Symptoms improved markedly during a six-month maternity leave and worsened upon return to work.

Findings:

  • Pulmonary function tests revealed a significant decline in FEV1 and PEFR upon returning to work.
  • Histamine challenge showed severe airway hyperresponsiveness (PC20 0.11 mg.ml-1) after returning to work.
  • Specific inhalation challenge with heated polypropylene (250°C) induced a late asthmatic reaction.

Implications:

  • Heated polypropylene is identified as a potential causative agent for occupational asthma.
  • This finding necessitates the inclusion of heated polypropylene in lists of occupational asthma triggers.
  • Further research into the specific components of heated polypropylene responsible for respiratory reactions is warranted.
