Summary

Severe hyponatremia can occur after hysteroscopic myomectomy using glycine irrigation. Early low sodium levels may be falsely low due to glycine

Background:

  • Hysteroscopic myomectomy is an increasingly common surgical procedure for uterine fibroids.
  • This technique utilizes large volumes of 1.5% glycine irrigation solution.
  • Hyponatremia is a potential complication, analogous to that seen after transurethral resection of the prostate.

Observation:

  • A case of severe hyponatremia following hysteroscopic myomectomy is presented.
  • The patient developed significant dilutional hyponatremia.
  • Glycine irrigation was used as the distension medium.

Findings:

  • Early hyponatremia was partly spurious, caused by the osmotic effect of glycine in the extracellular space.
  • As glycine was metabolized and entered cells, the hyponatremia became more representative of true hypo-osmolality.
