Globus symptoms are associated with pathological anxiety. Aggressive treatment of globus with tricyclic antidepressants like amitriptyline may lead to high treatment failure rates due to poor tolerability.

Background:

  • Globus is a common condition characterized by a sensation of a lump in the throat.
  • Globus has been anecdotally linked to anxiety disorders, but empirical evidence remains limited.

Purpose of the Study:

  • To investigate the association between psychiatric disorders, particularly anxiety, and globus symptoms.
  • To evaluate the efficacy and tolerability of amitriptyline in treating globus patients.

Main Methods:

  • Twenty-four outpatients with globus underwent psychiatric assessment using the 'Schedule for Affective Disorders and Schizophrenia-Lifetime Anxiety' version.
  • Patients were randomized to receive either amitriptyline or placebo in a double-blind manner.
  • Treatment outcomes were assessed using various psychometric scales and symptom inventories.

Main Results:

  • Nine patients met Diagnostic and Statistical Manual of Mental Disorders, Third Edition (DMS-III) criteria for past psychiatric disorders, with six diagnosed with panic disorder.
  • Nine out of 12 patients treated with amitriptyline discontinued the treatment, compared to two out of 12 in the placebo group.
  • Significant associations were found between pathological anxiety and globus symptoms.

Conclusions:

  • Globus symptoms are clinically and psychometrically associated with pathological anxiety.
  • Aggressive treatment of globus with tricyclic antidepressants may result in a high rate of treatment discontinuation and failure.
