Summary

General BCG vaccination for newborns is no longer recommended due to low tuberculosis incidence. Vaccination should be reserved for high-risk groups, such as children with infectious tuberculosis exposure.

Area of Science:

  • Public Health
  • Infectious Disease Epidemiology
  • Vaccinology

Context:

  • Follow-up studies in the Netherlands, Sweden, and Germany show a favorable tuberculosis situation.
  • Discontinuation of BCG vaccination in Sweden and West Germany since 1975 has not led to increased tuberculosis incidence.
  • Low tuberculosis incidence and infection risk observed in Germany.

Purpose:

  • To re-evaluate the necessity of general Bacillus Calmette-Guérin (BCG) vaccination for all newborns.
  • To determine if current tuberculosis epidemiology warrants a change in vaccination policy.
  • To identify specific high-risk groups that would benefit from BCG vaccination.

Summary:

  • Evidence from countries without widespread BCG vaccination indicates it is no longer generally warranted for babies.
  • The German Central Committee for Combatting Tuberculosis is considering discontinuing the recommendation for general newborn BCG vaccination.
  • BCG vaccination should be targeted towards enhanced-risk groups, including children of foreign parents and those in close contact with infectious tuberculosis cases.

Impact:

  • Potential shift in national tuberculosis prevention strategies.
  • Resource reallocation from general vaccination programs to targeted interventions.
  • Reduced public health burden associated with widespread vaccination while maintaining protection for vulnerable populations.
