Background:

Purpose of the Study:

  • To evaluate the efficacy and safety of pulse-dye laser lithotripsy for ureteric stones.
  • To compare laser lithotripsy with conventional ultrasound lithotripsy.
  • To determine the success rate and morbidity associated with endoscopic laser treatment.

Main Methods:

  • A prospective study involving 239 patients with 259 ureteric stones treated between May 1989 and December 1991.
  • Endoscopic insertion of a 7.2 ureteroscope (miniscope) without prior dilatation.
  • Stone fragmentation using a 504-nm pulse-dye laser (Candela laser lithotripter) with 30-60 mJ energy.

Main Results:

  • Complete stone fragmentation was achieved in 85% (202/239) of patients.
  • The average hospital stay was two days.
  • Morbidity included ureteral perforation in 1.3% (3/239), attributed to ureteroscopy, not laser lithotripsy.

Conclusions:

  • It offers a high success rate with acceptable morbidity.
  • Laser lithotripsy is now reserved for cases where extracorporeal shockwave lithotripsy (ESWL) has failed.
