    Summary

    Acromegaly patients show altered thyroid function, with reduced thyrotropin-releasing hormone (TRH) response and higher thyroxine levels. Pituitary surgery can normalize TRH response, indicating pituitary regeneration capacity.

    Background:

    • Acromegaly, a condition caused by excess growth hormone, can affect thyroid function.
    • Transsphenoidal hypophysectomy is a common surgical treatment for acromegaly.

    Purpose of the Study:

    • To investigate thyroid function in acromegaly patients before and after transsphenoidal hypophysectomy.
    • To assess the impact of surgery on the thyrotropin-releasing hormone (TRH) stimulation test response.

    Main Methods:

    • Studied thyroid parameters in 36 euthyroid, 1 hypothyroid, and 3 hyperthyroid acromegaly patients.
    • Administered a standard thyrotropin-releasing hormone (TRH) stimulation test (200 μg IV) before and after surgery.
    • Monitored serum TSH and total thyroxine levels.

    Main Results:

    • Untreated acromegaly patients exhibited a blunted TSH response to TRH compared to controls (7.1 mU/l vs. 12.5 mU/l).
    • Elevated total thyroxine and a high incidence of goiter (53%) were observed in acromegaly patients.
    • Post-surgery, 30% of euthyroid patients had subnormal TRH responses, but 7 patients showed normalization over time, suggesting pituitary regeneration.

    Conclusions:

    • Subnormal TRH responses in acromegaly patients, even post-surgery, do not necessarily predict hypothyroidism due to pituitary regeneration.
    • Acromegaly may be associated with an increased incidence of hyperthyroidism.
