Background:

  • Pulmonary lymphangioleiomyomatosis (LAM) is a rare neoplastic disease characterized by smooth muscle proliferation.
  • LAM predominantly affects women of childbearing age and leads to respiratory failure within a decade.

Observation:

  • High-resolution computed tomography (CT) reveals characteristic, nearly pathognomonic patterns for LAM.
  • Hormonal therapies have shown limited efficacy in treating LAM.

Findings:

  • Surgical or radiant oophorectomy (castration) is the most effective treatment for LAM, despite the rarity precluding randomized trials.
  • Early castration therapy has been associated with stable respiratory status in LAM patients.
