    Summary

    Systemic mastocytosis can cause generalized increased bone activity on scans, mimicking other bone diseases. This study suggests aberrant new-bone formation as the cause in a patient with mastocytosis.

    Background:

    • Systemic mastocytosis is a rare disorder characterized by abnormal mast cell accumulation.
    • Bone involvement is common in systemic mastocytosis, but its imaging features can be complex.
    • Differentiating mastocytosis-related bone changes from other conditions is crucial for accurate diagnosis and management.

    Observation:

    • A 45-year-old male patient with diagnosed systemic mastocytosis presented with generalized symmetric increased activity on bone imaging.
    • The observed bone scan findings were similar to those seen in renal osteodystrophy and secondary hyperparathyroidism.
    • Specific hand imaging revealed an absence of characteristic changes associated with secondary hyperparathyroidism.

    Findings:

    • Bone imaging in systemic mastocytosis can present with diffuse, symmetric hyperactivity.
    • These findings can be indistinguishable from other metabolic bone diseases like renal osteodystrophy.
    • Aberrant new-bone formation is proposed as the underlying mechanism for the intense bone scan activity in this case.

    Implications:

    • This case highlights the importance of considering systemic mastocytosis in the differential diagnosis of generalized increased bone activity.
    • Understanding the imaging patterns associated with mastocytosis can aid in distinguishing it from other bone disorders.
    • Further research into the mechanisms of bone remodeling in mastocytosis is warranted.
