    Summary

    Prophylactic isoniazid (INH) therapy for adolescents may cause liver enzyme elevations, suggesting potential hepatotoxicity. However, jaundice was absent, possibly due to INH inducing liver enzymes that process bilirubin.

    Background:

    • Isoniazid (INH) is recommended for adolescents with positive tuberculin reactions to prevent tuberculosis.
    • Hepatotoxicity is a known risk associated with INH in adults.

    Purpose of the Study:

    • To evaluate the effect of prophylactic isoniazid therapy on liver function in adolescents.
    • To investigate the mechanism behind the absence of jaundice despite elevated liver enzymes.

    Main Methods:

    • Liver function tests (SGPT) were performed on 178 adolescents before and after INH administration.
    • Hepatobiliary effects were studied in Sprague-Dawley rats receiving INH injections.

    Main Results:

    • Abnormal SGPT values were observed in 10% of adolescent patients, primarily within the first 10 weeks of INH therapy.
    • Rats treated with INH showed increased hepatic bilirubin glucuronyl transferase activity compared to controls.

    Conclusions:

    • Isoniazid may be hepatotoxic in adolescents, indicated by elevated SGPT levels.
    • Enzymatic induction of bilirubin metabolism is a potential explanation for the lack of jaundice in adolescents receiving INH.
