Area of Science:

  • Hepatobiliary Surgery
  • Surgical Oncology
  • Gastroenterology

Background:

  • Repeat hepatectomy for recurrent hepatocellular carcinoma (HCC) is debated.
  • Understanding survival and surgical risks is crucial.

Purpose of the Study:

  • To reevaluate survival and surgical risks of repeat hepatectomy for HCC.
  • To compare outcomes of repeat versus primary hepatectomy.

Main Methods:

  • Retrospective analysis of 21 patients undergoing curative repeat hepatectomy.
  • Comparison with 253 patients who had primary hepatectomy.
  • Analysis of preoperative liver function tests and surgical outcomes.

Main Results:

  • Second hepatectomy showed improved liver function (higher indocyanine green excretion rate).

Related Experiment Videos

  • No significant differences in blood loss, operation time, or complications between first and second hepatectomies.
  • Repeat hepatectomy patients had shorter hospital stays and similar survival and disease-free survival rates compared to primary hepatectomy patients.
  • Conclusions:

    • Curative repeat hepatectomy is a safe and effective treatment for recurrent HCC.
    • It offers comparable oncological outcomes to primary hepatectomy.
