Summary

Most pregnant women with ulcerative colitis (UC) have healthy pregnancies. However, severe UC during pregnancy may necessitate surgery, as seen in a case requiring sub-total colectomy, yet resulting in a healthy infant.

Area of Science:

  • Gastroenterology and Obstetrics
  • Inflammatory Bowel Disease (IBD) research
  • Perinatal care for chronic conditions

Background:

  • Ulcerative colitis (UC) is a chronic inflammatory bowel disease.
  • Pregnancy in women with UC is generally favorable.
  • Disease management during pregnancy requires careful consideration.

Observation:

  • A pregnant woman developed severe ulcerative colitis early in gestation.
  • Initial treatment with steroid enemas provided temporary relief.
  • The patient experienced a relapse, progressing to severe colitis.

Findings:

  • A sub-total colectomy was performed at 26 weeks gestation due to severe UC.
  • The surgical intervention was complicated by intra-abdominal sepsis and abscess formation.

Related Experiment Videos

  • The patient successfully carried the pregnancy to 32 weeks gestation.
  • Implications:

    • This case highlights the potential for severe UC exacerbations during pregnancy.
    • Surgical management of severe UC in pregnancy, though complex, can lead to a successful outcome.
    • Further research into optimal UC management strategies during pregnancy is warranted.
