    Summary

    Silver nitrate prophylaxis significantly reduced gonococcal ophthalmia neonatorum risk to less than 2%. Topical sulfonamides treated chlamydial cases effectively, while penicillin and antibiotics treated gonococcal infections.

    Background:

    • Ophthalmia neonatorum (ON) is a significant neonatal infection.
    • Review of ON cases diagnosed at Grady Memorial Hospital from 1967-1973.
    • Evaluation of prophylaxis and treatment efficacy.

    Observation:

    • 302 cases reviewed: 43 gonococcal, 86 chlamydial, 3 mixed, 31 staphylococcal, 5 chemical.
    • Silver nitrate prophylaxis was standard practice.
    • Gonococcal ON peaked in Q3, chlamydial in Q4.
    • Chlamydial ON more prevalent in Black infants; others showed no racial disparity.

    Findings:

    • Gonococcal ON linked to prolonged ruptured fetal membranes.
    • Clinical diagnosis alone was insufficient for definitive etiology.
    • Credé prophylaxis reduced gonococcal ON risk to <2% in infants of infected mothers.
    • Topical sulfonamides effective for chlamydial ON.
    • Parenteral penicillin and topical antibiotics effective for gonococcal ON.

    Implications:

    • Effective prophylaxis and targeted therapies are crucial for managing ON.
    • Understanding etiologic distribution and risk factors informs prevention strategies.
    • Further research needed for definitive clinical diagnosis of ON subtypes.
