    Summary

A rare case of recurrent thrombocytopenic purpura was treated. Surgical removal of a splenunculus (a small spleen fragment) led to a lasting remission of the patient's condition.

    Area of Science:

    • Hematology
    • Nuclear Medicine
    • Surgical Pathology

    Background:

    • Idiopathic thrombocytopenic purpura (ITP) can present with severe, intermittent thrombocytopenia.
    • Splenectomy is a common treatment for refractory ITP, but recurrence can occur.
    • Diagnostic imaging plays a role in identifying accessory splenic tissue.

    Observation:

    • A 13-year-old boy initially presented with thrombocytopenic purpura, treated with cortisone.
    • Despite splenectomy, the patient experienced recurrent thrombocytopenia due to a previously undetected splenunculus.
    • Advanced imaging techniques, including gamma-camera scanning, identified the accessory spleen.

    Findings:

    • The splenunculus was successfully removed via splenunculectomy.
    • Post-surgical splenunculectomy resulted in a rapid and sustained remission of thrombocytopenia.
    • This case highlights the importance of thorough imaging in diagnosing ITP recurrence.

    Implications:

    • Splenunculectomy can be an effective treatment for ITP recurrence caused by accessory spleens.
    • Nuclear medicine imaging is crucial for detecting splenunculi in patients with ITP.
    • Accurate diagnosis and targeted surgical intervention are key to managing complex ITP cases.
