    Summary

    Intravenous nitroglycerin effectively lowered blood pressure and reduced myocardial oxygen demand in hypertensive patients during heart surgery. This treatment also improved electrocardiogram indicators of myocardial ischemia.

    Purpose of the Study:

    • To evaluate the efficacy and safety of intravenous nitroglycerin in acutely hypertensive patients undergoing coronary-artery surgery.
    • To assess the impact of nitroglycerin on hemodynamic parameters and myocardial oxygen demand.

    Main Methods:

    • Twenty acutely hypertensive patients received intravenous nitroglycerin during coronary-artery surgery.
    • Hemodynamic measurements, including blood pressure, central venous pressure, and pulmonary capillary wedge pressure, were recorded.
    • Myocardial oxygen demand indices (rate-pressure product, tension-time index) and electrocardiogram (ST-segment depression) were monitored.

    Main Results:

    • Nitroglycerin significantly decreased systolic, diastolic, and mean arterial blood pressures, central venous pressure, pulmonary capillary wedge pressure, systemic vascular resistance, and left ventricular stroke work index.
    • Cardiac index, stroke index, and heart rate remained unchanged.
    • Myocardial oxygen demand indices were significantly reduced, and 50% of patients showed improvement in ST-segment depression.

    Conclusions:

    • Intravenous nitroglycerin can be safely administered during operation in patients with hypertension undergoing coronary-artery surgery.
    • Nitroglycerin effectively reduces myocardial oxygen demand and alleviates myocardial ischemia in this setting.
    • Nitroglycerin is a valuable tool for managing hypertensive crises and myocardial ischemia during cardiac procedures.
