    Summary

    Acoustic neurinomas can cause inner ear damage by obstructing blood flow, leading to nerve and stria vascularis degeneration. Remarkably, sensory cells may remain preserved despite severe nerve damage.

    Background:

    • Acoustic neurinomas, benign tumors arising from the vestibulocochlear nerve, can cause progressive hearing loss and tinnitus.
    • Tumor growth within the internal auditory meatus may lead to chronic partial obstruction of blood circulation to the inner ear.
    • Understanding the histopathological consequences of such vascular compromise is crucial for managing audiovestibular symptoms.

    Observation:

    • Histopathological examination of temporal bones from three acoustic neurinoma cases was performed.
    • Inner ear lesions were evaluated and compared with contralateral, unaffected ears.
    • Specific focus was placed on degenerative changes resulting from chronic partial vascular obstruction.

    Findings:

    • Degeneration of cochlear nerve fibers and spiral ganglion cells was a primary finding.
    • Pathological changes included degeneration of the stria vascularis and tectorial membrane.
    • Fibrosis and ossification of a semicircular canal were observed in affected ears.
    • Notably, sensory cells showed relatively good preservation despite severe neural degeneration.

    Implications:

    • These findings highlight the vulnerability of neural and vascular structures within the inner ear to acoustic neurinoma-induced ischemia.
    • The preservation of sensory cells suggests potential therapeutic targets for mitigating hearing loss.
    • Further research into the mechanisms of ischemia and potential neuroprotective strategies is warranted.
