Summary

Routine preoperative intravenous urography (IVU) is not necessary before colorectal surgery. Careful surgical technique, not IVU, is key to preventing ureteral damage during colon and rectal operations.

Background:

  • Iatrogenic damage to the urogenital tract can occur during colorectal surgery.
  • Preoperative intravenous urography (IVU) has been used to identify potential risks.

Purpose of the Study:

  • To investigate the value of preoperative intravenous urography (IVU) in preventing iatrogenic damage to the urogenital tract in patients undergoing colon or rectal surgery.

Main Methods:

  • Retrospective analysis of 424 patients (236 female, 188 male) who underwent colon or rectal surgery between April 1988 and March 1992.
  • Preoperative IVU was performed in 65.8% of patients.
  • Data on intraoperative complications, specifically ureteral damage, were collected and analyzed.

  • Preoperative IVU revealed abnormalities in 26.9% of cases, but only 6.8% were directly related to colorectal disease.
  • Six patients had abnormal findings on other diagnostic procedures despite unremarkable IVUs.

  • No ureteral damage occurred in patients who did not undergo preoperative IVU.

Conclusions:

  • Preoperative IVU is not a decisive factor in preventing intraoperative ureteral damage.
  • Careful intraoperative dissection and visualization of the ureter are the most important factors.
  • Routine preoperative IVU is not recommended for patients undergoing colon or rectal surgery.
