    Summary

    Radiotherapy for verrucous carcinoma did not lead to anaplastic transformation in this study. This treatment showed favorable disease control and survival rates compared to surgery, suggesting it is a viable option.

    Background:

    • Verrucous carcinoma is a subtype of squamous cell carcinoma.
    • Radiotherapy for verrucous carcinoma has been historically linked to anaplastic transformation and poor treatment outcomes.

    Purpose of the Study:

    • To evaluate the efficacy and safety of radiotherapy as a primary treatment for verrucous carcinoma.
    • To determine if radiotherapy is associated with anaplastic transformation in verrucous carcinoma patients.

    Main Methods:

    • Retrospective review of 6 patients with verrucous carcinoma treated with radiotherapy as primary treatment.
    • Comparison of outcomes with 5 patients treated with surgery.
    • Literature review of verrucous carcinoma radiotherapy cases.

    Main Results:

    • No anaplastic transformation was observed in patients treated with radiotherapy.
    • Favorable disease control was achieved in the radiotherapy group.
    • Four out of six patients receiving radiotherapy were alive and disease-free (NED) at 1-4 years post-treatment.
    • Literature and case review did not substantiate an increased risk of anaplastic transformation with radiotherapy.

    Conclusions:

    • Radiotherapy can be a safe and effective primary treatment for verrucous carcinoma.
    • The risk of anaplastic transformation from radiotherapy for verrucous carcinoma may be lower than previously suggested.
    • Verrucous carcinoma should be managed similarly to other squamous cell carcinomas based on site and stage.
