[Decrease in cerclage incidence in multiple pregnancies by vaginal ultrasound monitoring]

Summary

Prophylactic cerclage does not improve pregnancy duration in multiple pregnancies. Vaginal ultrasound monitoring effectively diagnoses cervical insufficiency, suggesting prophylactic cerclage is unnecessary.

Area of Science:

  • Obstetrics and Gynecology
  • Maternal-Fetal Medicine

Background:

  • Cervical insufficiency is a common complication in multiple pregnancies.
  • It is often an indication for cerclage surgery.

Purpose of the Study:

  • To evaluate the efficacy of prophylactic cerclage versus selective cerclage based on vaginosonographic monitoring in multiple pregnancies.
  • To determine if prophylactic cerclage improves fetal outcomes.

Main Methods:

  • Prospective vaginosonographic monitoring in 35 multiple pregnancies.
  • Comparison with 41 multiple pregnancies receiving prophylactic cerclage.
  • Assessment of pregnancy duration, preterm contractions, and premature rupture of membranes.

Main Results:

  • No significant difference in pregnancy duration between groups.

Related Experiment Videos

  • Lower frequency of preterm contractions in the non-prophylactic cerclage group.
  • Higher frequency of premature rupture of membranes in the non-prophylactic cerclage group.
  • Conclusions:

    • Prophylactic cerclage does not improve fetal outcomes in multiple pregnancies.
    • Vaginosonographic monitoring is sufficient for early diagnosis and timely cerclage when indicated.
