Summary

Prophylactic anticonvulsant treatment is generally unnecessary for chronic subdural hematoma (SDH) patients, as epilepsy incidence is low. However, alcoholic patients with SDH face a higher risk of seizures.

Area of Science:

  • Neurosurgery
  • Neurology
  • Clinical Medicine

Background:

  • Chronic subdural hematoma (SDH) is a significant clinical concern in neurosurgery.
  • Seizures are a potential complication of SDH, necessitating evaluation of prophylactic treatment strategies.
  • Previous studies have yielded varied conclusions regarding the necessity of anticonvulsants in SDH management.

Purpose of the Study:

  • To determine the incidence of seizures in patients with chronic subdural hematoma (SDH).
  • To evaluate the necessity of prophylactic anticonvulsant treatment in patients with chronic SDH.
  • To compare seizure rates in a retrospective patient series with existing literature data.

Main Methods:

  • Retrospective analysis of 143 adult patients treated for chronic subdural hematoma (SDH) between 1979 and 1991.
  • Literature review and inclusion of 879 additional cases for comparative analysis.
  • Statistical comparison of epilepsy incidence before and after surgical intervention.

Main Results:

  • The overall incidence of epilepsy in chronic SDH patients was found to be low.
  • Pre-operative epilepsy rates were 5.6% in the studied series and 4.3% in the literature.
  • Post-operative epilepsy rates decreased to 3% in the series and 1.8% in the literature.

Conclusions:

  • Routine prophylactic anticonvulsant administration is not indicated for most patients with chronic subdural hematoma (SDH).
  • Alcoholic patients with chronic SDH represent a distinct subgroup with a significantly higher risk of epilepsy.
  • Further research may be warranted to refine treatment guidelines for specific high-risk populations, such as alcoholics.
