Summary

Ticlopidine is an antiplatelet drug more effective than aspirin for stroke prevention. While beneficial for unstable angina and graft patency, its use is limited by cost and potential neutropenia.

Area of Science:

  • Pharmacology and Therapeutics
  • Cardiovascular Medicine
  • Clinical Trials

Background:

  • Ticlopidine is a novel antiplatelet agent with a unique mechanism of action.
  • Existing antiplatelet therapies have varying efficacy and safety profiles.

Purpose of the Study:

  • To comprehensively review ticlopidine's pharmacology, pharmacokinetics, clinical efficacy, and toxicity.
  • To compare ticlopidine's efficacy against other antiplatelet agents.
  • To provide a therapeutic recommendation for ticlopidine.

Main Methods:

  • Literature search of English-language articles from 1987 to January 1993 via MEDLINE.
  • Prioritization of randomized, blind, controlled studies.
  • Review of clinical trials focusing on study design, efficacy, and toxicity.

Main Results:

  • Ticlopidine demonstrated superior efficacy to aspirin in preventing stroke or death in a major trial.
  • The drug reduced vascular death and myocardial infarction in unstable angina patients.
  • It also maintained venous graft patency post-coronary artery bypass grafting.
  • Potential benefits in diabetic microangiopathy and peripheral vascular disease require further investigation.
  • Common adverse effects include gastrointestinal issues; severe neutropenia occurs in approximately 2.3% of patients.

Conclusions:

  • Ticlopidine serves as a viable alternative to aspirin for stroke prevention, particularly for aspirin-intolerant or non-responsive patients.
  • High cost and hematologic adverse effects, such as neutropenia, restrict its first-line use.
