Leukemic infiltration of the optic nerve is rare in acute lymphoblastic leukemia (T-ALL). A-scan echography is crucial for early diagnosis of ocular involvement, aiding timely treatment and visual recovery.

Area of Science:

  • Ophthalmology
  • Hematology
  • Medical Imaging

Background:

  • Leukemic infiltration of the optic nerve is an uncommon presentation.
  • Acute lymphoblastic leukemia (T-ALL) can manifest with extramedullary relapses.

Observation:

  • Two adult patients with T-ALL presented with isolated leukemic eye involvement.
  • Computed tomography (CT) and magnetic resonance imaging (MRI) scans were inconclusive for ocular disease.

Findings:

  • A-scan echography successfully diagnosed leukemic infiltration of the optic nerve in both patients.
  • Early radiotherapy led to visual acuity recovery in one patient.

Implications:

  • A-scan echography is the most sensitive diagnostic tool for detecting ocular leukemia.
