Summary

Total prostatectomy offers excellent long-term disease-free survival for select patients, comparable to age-matched survival rates. While risks exist, complications are minimal, manageable, and outweighed by the benefits of avoiding advanced prostate cancer.

Area of Science:

  • Urology
  • Oncology

Background:

  • Hormone-refractory metastatic prostate cancer presents significant morbidity and mortality.
  • Effective treatment options for advanced prostate cancer are limited, leading to poor outcomes.

Purpose of the Study:

  • To evaluate the long-term disease-free survival rates after total prostatectomy in properly selected patients.
  • To assess the risks and benefits of total prostatectomy in comparison to the progression of prostate cancer.

Main Methods:

  • Observational study analyzing disease-free survival up to 30 years post-total prostatectomy.
  • Review of potential complications, their management, and overall patient outcomes.

Main Results:

  • Total prostatectomy provides disease-free survival rates comparable to expected survival in similarly aged men for up to 30 years.
  • Patients undergoing total prostatectomy face a very small risk of long-term complications or mortality, which are generally manageable.

Conclusions:

  • Total prostatectomy is a safe and effective treatment for properly selected prostate cancer patients.
  • The procedure offers excellent long-term disease-free survival with manageable risks, significantly improving outcomes compared to hormone-refractory metastatic prostate cancer.
