Summary

Macular ischemia on fluorescein angiography may predict poor visual outcomes in posterior uveitis patients. Immunosuppressive therapy should be used cautiously in these cases.

Background:

  • Cystoid macular edema is a common cause of visual morbidity in posterior uveitis, typically responding to immunosuppression.
  • A subset of posterior uveitis patients exhibit poor visual outcomes despite adequate immunosuppressive therapy.

Observation:

  • A retrospective study analyzed 345 angiograms from 135 patients with active non-occlusive retinal vasculitis.
  • Macular ischemia was identified in 12 patients (4 Behçet's disease, 4 sarcoidosis, 4 idiopathic retinal vasculitis).

Findings:

  • Over a 36-month follow-up, visual acuity did not improve in 4 patients.
  • Visual acuity decreased by an average of three Snellen lines in the remaining 8 patients.

Implications:

  • Macular ischemia on fluorescein angiography may predict poor visual prognosis in posterior uveitis.
  • Caution is advised when prescribing immunosuppressive therapy for patients with identified macular ischemia.
