Background:

  • Valproate's antimanic effects are linked to serum concentrations above 50 µg/ml.
  • Oral loading of valproate can achieve therapeutic levels rapidly with few side effects.

Purpose of the Study:

  • To assess the safety and efficacy of valproate oral loading for acute mania.
  • To provide further data on rapid valproate administration in manic patients.

Main Methods:

  • 13 patients with acute mania received valproate at 20 mg/kg/day.
  • Valproate serum concentrations and clinical response were monitored.
  • Patients received treatment for at least 5 days, often with concurrent psychotropics.

Main Results:

  • Therapeutic valproate serum concentrations (mean 88 ± 25 µg/ml) were achieved by day 2-3.
