Background:

  • Angiotensin converting enzyme (ACE) inhibitors are widely prescribed for hypertension and heart failure.
  • While generally safe, ACE inhibitors can cause severe adverse effects, including hyperkalemia.
  • Life-threatening hyperkalemia is a rare but serious complication.

Observation:

  • A patient with heart failure and renal insufficiency developed acute hyperkalemia.
  • The hyperkalemia persisted despite standard treatments aimed at intracellular potassium shift.
  • Azotemia did not increase concurrently with the hyperkalemia.

Findings:

  • This case required hemodialysis for hyperkalemia management, a treatment not previously reported for ACE inhibitor-induced hyperkalemia.
  • The lack of response to conventional therapy and absence of worsening azotemia suggested a primary defect in potassium regulation.
