Symptomless hypoglycaemia

A T Andreasen, C Maraspini

South African Medical Journal = Suid-Afrikaanse Tydskrif Vir Geneeskunde
    |August 7, 1976
    PubMed
    Summary

This case study highlights a pregnant woman with severe hypoglycemia (blood glucose <10 mg/100 ml) who showed no symptoms. Her Rh-negative status did not appear to influence this unusual asymptomatic hypoglycemic state.

    Area of Science:

    • Obstetrics and Gynecology
    • Endocrinology
    • Hematology

    Background:

    • Pregnancy presents unique physiological challenges, including altered glucose metabolism.
    • Hypoglycemia, or low blood glucose, typically manifests with distinct clinical symptoms.
    • Rh-negative incompatibility is a significant concern during pregnancy.

    Observation:

    • A pregnant patient presented with critically low random blood glucose levels (<10 mg/100 ml).
    • Despite glucose concentrations as low as 7 mg/100 ml, the patient exhibited no signs or symptoms of hypoglycemia.
    • The patient's Rh-negative status was noted but appeared unrelated to the observed asymptomatic hypoglycemia.

    Findings:

    • Documented an unprecedented case of asymptomatic severe hypoglycemia in a pregnant woman.
    • Demonstrated a profound dissociation between extremely low blood glucose levels and the absence of typical hypoglycemic symptoms.
    • Found no correlation between the patient's Rh-negative condition and her asymptomatic hypoglycemic state.

    Implications:

    • Challenges current understanding of glucose homeostasis and symptom manifestation during pregnancy.
    • Suggests potential for novel physiological adaptations or compensatory mechanisms in certain individuals.
    • Underscores the need for further research into atypical presentations of metabolic disorders in obstetric patients.
