    Summary

    This study reports a peripheral odontogenic fibroma with granular cells, noting similarities to other granular cell tumors. It suggests ectomesenchymal influence may explain epithelial strand formation in these fibromas.

    Background:

    • Peripheral odontogenic fibroma (POF) is a benign odontogenic tumor.
    • Granular cell changes are observed in various oral lesions, including POF.
    • Epithelial downgrowth is a characteristic feature of some POFs.

    Observation:

    • A rare case of POF with significant aggregates of large granular cells is presented.
    • These granular cells exhibit morphological similarities to those found in granular cell myoblastoma and congenital epulis.
    • Deep, double-stranded extensions of the basal layer of the overlying gingival epithelium were frequently observed.

    Findings:

    • The granular cells in this POF case resemble those in other documented granular cell lesions.
    • The observed epithelial strands in POF closely mimic the morphology of the tumor itself.
    • Similar basal cell prolongations are noted in other jaw lesions, suggesting a common underlying mechanism.

    Implications:

    • A hypothesis is proposed suggesting residual ectomesenchymal influence as the driver for basal cell proliferations in POF.
    • This influence may mimic early embryonic dental development, potentially explaining the histogenesis of odontogenic epithelial strands.
    • Understanding this mechanism could provide insights into the development of various odontogenic and jaw lesions.
