Summary

Actinomyces infections of the tear drainage system are often misdiagnosed and mistreated. Surgical removal of concretions, alongside antibiotics, is essential for curing Actinomyces canalicular disease.

Area of Science:

  • Ophthalmology
  • Microbiology

Background:

  • Actinomyces species and Arachnia propionica can cause canalicular infections.
  • These infections are often chronic and misdiagnosed, leading to prolonged inappropriate treatment.

Purpose of the Study:

  • To describe the clinical presentation and treatment outcomes of canalicular infections caused by Actinomyces.
  • To emphasize the importance of accurate diagnosis and appropriate management.

Main Methods:

  • Retrospective review of 7 cases of canalicular involvement with Actinomyces over a 5-year period.
  • Analysis of clinical data, microbiological findings, and treatment responses.

Main Results:

  • All 7 patients had unilateral canalicular involvement with patent lacrimal drainage.
  • Actinomyces species (5 cases) and Arachnia propionica (2 cases) were identified in curettings, often with mixed bacterial flora.
  • Patients experienced diagnostic delays and inappropriate treatments for recurrent symptoms.

Conclusions:

  • Surgical evacuation of concretions is crucial for curing Actinomyces canalicular disease.
  • Medical therapy alone is insufficient, despite the broad-spectrum antibiotic sensitivity of Actinomyces.
  • Early and accurate diagnosis is vital to prevent prolonged morbidity.
