Summary

Carotid surgery is safe for elderly patients over 75, with similar risks to younger patients. Elderly patients had higher rates of asymptomatic carotid stenosis and bilateral disease, indicating a role for carotid endarterectomy in this population.

Area of Science:

  • Vascular Surgery
  • Geriatric Medicine
  • Neurology

Background:

  • Carotid endarterectomy is a common procedure to prevent stroke.
  • The safety and efficacy in elderly patients (75 years and older) require specific evaluation.
  • Previous studies have not extensively focused on the outcomes of carotid surgery in this demographic.

Purpose of the Study:

  • To evaluate the role and outcomes of carotid endarterectomy in patients aged 75 years and older.
  • To compare the risk profile, indications, and outcomes of carotid surgery in elderly versus younger patients.
  • To assess the long-term follow-up data for elderly patients undergoing carotid endarterectomy.

Main Methods:

  • Retrospective review of 912 carotid endarterectomies performed between 1987 and 1990.

Related Experiment Videos

  • Comparison of 151 elderly patients (group A, ≥75 years) with 655 younger patients (group B, <75 years).
  • Analysis of patient demographics, risk factors, indications for surgery, operative outcomes, and follow-up data.
  • Main Results:

    • Elderly patients had a similar risk profile, except for less frequent coronary heart disease (25% vs 44%).
    • Higher rates of prophylactic endarterectomy for stenosis (41% vs 30%) and bilateral carotid disease (59% vs 40%) were observed in the elderly group.
    • Operative mortality was slightly higher in the elderly group (2.5% vs 1.5%), with cardiac causes being the most frequent (60%).

    Conclusions:

    • Carotid endarterectomy is a viable option for elderly patients (≥75 years) with acceptable operative risks.
    • Elderly patients present with different indications and anatomical findings, such as higher rates of asymptomatic stenosis and bilateral disease.
    • The study supports the consideration of carotid surgery in carefully selected elderly patients to prevent stroke.
