Primary hyperparathyroidism (pHPT) significantly impacts very old patients, often presenting with severe neuropsychiatric symptoms. Early detection and surgical intervention can lead to substantial improvements in cognitive function.

Background:

  • Primary hyperparathyroidism (pHPT) is a common endocrine disorder.
  • Its clinical presentation in very elderly individuals (≥75 years) is not well-characterized.
  • Neuropsychiatric manifestations are increasingly recognized in pHPT.

Purpose of the Study:

  • To evaluate the clinical presentation and biological features of pHPT in very old subjects.
  • To highlight the prevalence and severity of neuropsychiatric disorders in this population.
  • To assess the impact of treatment on these symptoms.

Main Methods:

  • Retrospective analysis of 30 patients aged 75 years or older diagnosed with pHPT.
  • Diagnosis confirmed by biological signs, parathyroid imaging, or surgical exploration.

  • Evaluation of clinical symptoms, serum calcium, parathyroid hormone levels, and treatment outcomes.
  • Main Results:

    • 87% presented with general symptoms, 83% with neuromuscular symptoms, and 73% with neuropsychiatric manifestations.
    • Confusion and depression were common (30% and 27%).
    • Serum calcium was normal in 13% and ≤110 mg/L in 47%; no correlation with mental changes. Parathyroid hormone was abnormal in 93%.
    • Surgery improved all 13 patients; ethanol injection cured 50% of 10 patients.

    Conclusions:

    • pHPT frequently causes significant neuropsychiatric disorders in the very old.
    • These disturbances can improve with surgical treatment.
    • Clinicians should systematically screen elderly patients with cognitive impairment for pHPT.
