    Background:

    • Zenker's diverticulum is a rare condition involving an outpouching in the pharynx.
    • Upper gastrointestinal bleeding is a serious medical emergency.

    Observation:

    • A patient presented with massive upper gastrointestinal bleeding.
    • The bleeding originated from an ulcer within a Zenker's diverticulum.

    Findings:

    • The ulcer within the Zenker's diverticulum was identified as the source of bleeding.
    • Topical effects of aspirin intake were determined to be the most probable cause of the ulcer and subsequent bleeding.

    Implications:

    • This case highlights a rare but significant cause of upper gastrointestinal bleeding.
    • Awareness of this complication is crucial for clinicians managing patients with Zenker's diverticulum, especially those on aspirin therapy.
