    Background:

    • 10% soybean oil emulsion (Intralipid) is widely used for parenteral nutrition.
    • It is generally considered safe for delivering caloric intake via peripheral veins.
    • Fat overload syndrome (FOS) was a known complication of older lipid emulsions.

    Observation:

    • A pediatric patient initially tolerated intravenous soybean oil emulsion well.
    • The child subsequently developed a sudden, severe intolerance to the infusion.
    • Symptoms were consistent with fat overload syndrome.

    Findings:

    • This case represents the first reported instance of fat overload syndrome with 10% soybean oil emulsion.
    • The patient experienced a life-threatening adverse reaction to the standard lipid infusion.
    • The intolerance appeared suddenly after a period of successful administration.

    Implications:

    • Clinicians should be aware of the potential for fat overload syndrome even with modern soybean oil emulsions.
    • Early recognition and management of FOS are crucial in pediatric patients receiving Intralipid.
    • Further research may be needed to understand the mechanisms and risk factors for FOS in this context.
