Summary

A patient experienced femoral thrombophlebitis and recurrent pulmonary emboli after spinal surgery. This led to pulmonary hypertension, cor pulmonale, and ultimately, fatal congestive heart failure.

Area of Science:

  • Medical Case Study
  • Cardiovascular Medicine
  • Neurosurgery Complications

Background:

  • A 53-year-old male underwent Harrington nail implantation for a lumbar vertebral issue.
  • The patient developed femoral thrombophlebitis post-surgery.

Observation:

  • Recurrent pulmonary emboli were noted from 1983 until 1987.
  • The patient experienced a hypotensive episode during right ventricular catheterization, leading to ventricular fibrillation.
  • Autopsy confirmed multiple recurrent thromboemboli to the lungs.

Findings:

  • Chronic thromboemboli led to significant occlusion of pulmonary arterial circulation.
  • Pulmonary hypertension developed secondary to recurrent embolization.
  • Cor pulmonale and congestive heart failure ensued, contributing to the patient's decline.

Implications:

  • This case highlights a rare but severe long-term complication of spinal instrumentation.
  • It underscores the importance of monitoring for thromboembolic events post-surgery.
  • The progression to pulmonary hypertension and cor pulmonale demonstrates the critical impact of unresolved thromboemboli on cardiopulmonary health.
