[Pseudo-hallucinations after long-term lithium treatment]

Summary

Lithium carbonate prophylaxis for recurrent depression can cause visual and auditory pseudohallucinations. These perceptual disturbances returned when lithium was restarted, indicating a direct link.

Background:

Recurrent depressive episodes necessitate long-term mood stabilization.
Lithium carbonate is a well-established prophylactic treatment for bipolar disorder and recurrent depression.

Observation:

A 58-year-old woman on long-term lithium carbonate prophylaxis developed increasing visual and auditory pseudohallucinations.
These perceptual disturbances resolved upon discontinuation of lithium medication.

Findings:

The patient's pseudohallucinations recurred when lithium carbonate was reintroduced at therapeutic blood levels.
The recurrence of symptoms at therapeutic lithium levels strongly suggests a causal relationship.

Implications:

Lithium-induced pseudohallucinations can mimic psychotic symptoms, necessitating careful differential diagnosis.
Clinicians should consider medication side effects, particularly lithium toxicity or idiosyncratic reactions, in patients presenting with new-onset hallucinations.
Discontinuation and cautious re-challenge with lithium can help confirm drug-induced perceptual disturbances.
