Area of Science:

  • Otolaryngology
  • Head and Neck Surgery
  • Surgical Complications

Background:

  • Elective tracheotomy is a common procedure prior to extensive head and neck resections.
  • Resection of the floor of the mouth is a complex surgical intervention often requiring airway management.
  • The potential complications of tracheotomy are well-documented, but novel issues can still arise.

Observation:

  • A patient undergoing elective tracheotomy for floor of mouth resection developed unexpected postoperative complications.
  • These complications were life-threatening and directly related to the tracheotomy procedure.
  • Specifically, surgical emphysema of the tracheal mucosa was observed.

Findings:

  • Surgical emphysema of the tracheal mucosa, a novel complication, was identified following elective tracheotomy.
  • This finding represents a previously undescribed adverse event associated with tracheotomy.
  • The emphysema contributed to the life-threatening nature of the postoperative complications.

Implications:

  • This case underscores the importance of vigilance for rare complications even with routine procedures.
  • Awareness of surgical emphysema of the tracheal mucosa may inform preventative strategies and management.
  • Further research into the pathophysiology and incidence of this complication is warranted.
