Area of Science:

  • Dermatology
  • Infectious Diseases
  • Orthopedics

Background:

  • Genital ulcers are common presentations in dermatology and infectious disease clinics.
  • Chancroid is a sexually transmitted infection that causes genital ulcers.
  • Differential diagnosis of genital ulcers requires thorough investigation.

Observation:

  • A female patient presented to a skin outpatient department with a genital sore.
  • The lesion was initially managed as a case of chancroid.
  • Further diagnostic workup was initiated due to persistent or atypical features.

Findings:

  • The genital sore was identified as the external opening of a sinus tract.
  • The sinus tract originated from underlying chronic osteomyelitis.
  • The affected bone was the pubic symphysis.

Implications:

  • This case highlights the importance of considering non-sexually transmitted causes for genital ulcers.
  • Chronic osteomyelitis of the pubic symphysis can present with unusual cutaneous manifestations.
  • Accurate diagnosis is crucial for effective management of complex infections.
