Summary

Urinary incontinence and micturitional symptoms are common in progressive supranuclear palsy (PSP). This neurological condition frequently causes bladder storage issues, more so than Parkinson's disease.

Background:

  • Progressive supranuclear palsy (PSP) is a rare neurodegenerative disease.
  • Micturitional dysfunction is a recognized but understudied symptom in PSP.

Purpose of the Study:

  • To investigate the prevalence and characteristics of micturitional symptoms in patients with PSP.
  • To compare the urinary dysfunction in PSP with Parkinson's disease and striato-nigral degeneration (SND).

Main Methods:

  • Detailed micturitional histories were collected from nine PSP patients.
  • Urodynamic studies were performed in six patients.
  • Motor unit analysis of the external sphincter was conducted in four patients.

  • 89% of PSP patients experienced micturitional symptoms, with 7 out of 9 reporting urinary incontinence.
  • Urodynamic findings included high average residual urine (100 ml), detrusor hyperreflexia, low compliance, and detrusor-sphincter dyssynergia.
  • Neurogenic changes were observed in the external sphincter of two out of four patients.

Conclusions:

  • Severe micturitional disturbances, particularly in the urinary storage phase, are common in PSP, comparable to SND and more frequent than in Parkinson's disease.
  • Supranuclear pelvic and pudendal nerve dysfunction are likely the primary causes of micturitional issues in PSP.
