Summary

This study presents a successful kidney transplant in a patient with end-stage renal failure due to bladder exstrophy. The novel approach using continent external urinary diversion offers a good quality of life and renal function.

Background:

  • Bladder exstrophy is a congenital condition that can lead to end-stage renal failure.
  • Ileal conduit diversion is a common surgical option for urinary management in bladder exstrophy patients.
  • End-stage renal failure necessitates renal replacement therapy, such as kidney transplantation.

Observation:

  • A 29-year-old male with a history of bladder exstrophy and ileal conduit diversion presented with end-stage renal failure.
  • The patient underwent bilateral nephrectomy and a continent external urinary diversion.
  • Subsequently, a cadaveric kidney transplant was performed into the right iliac fossa.

Findings:

  • At 18 months post-transplant, the patient exhibited a serum creatinine level of 1.2 mg/dL, indicating good renal function.

  • The patient remained completely dry, managing with 4-5 daily catheterizations.
  • This demonstrates the viability of combining renal transplantation with continent external urinary diversion.
