Summary

Doppler ultrasonography (US) shows elevated resistive index (RI) in established diabetic nephropathy but is often normal in early stages. Renal Doppler US accurately indicates renal function but offers no significant advantage over conventional tests for diabetic kidney disease.

Background:

  • Diabetic kidney disease (DKD) is a major complication of diabetes mellitus.
  • Early detection and monitoring of DKD are crucial for patient outcomes.
  • Conventional methods for evaluating DKD include clinical and laboratory tests.

Purpose of the Study:

  • To compare the efficacy of Doppler ultrasonography (US) with conventional clinical and laboratory tests in evaluating diabetic renal disease.
  • To assess the utility of renal resistive index (RI) measurement in diagnosing and monitoring DKD.

  • Ninety-eight patients with diabetes mellitus underwent renal Doppler US for resistive index (RI) measurement.
  • US data were compared with clinical variables and follow-up data.

  • Main Results:

    • Mean RI was not significantly different between patients without nephropathy (0.62 ± 0.09) and those with early nephropathy (0.64 ± 0.09).
    • Patients with established nephropathy had a significantly higher mean RI (0.83 ± 0.11, P < .001).
    • Renal RI correlated strongly with serum creatinine (r = .64) and creatinine clearance (r = .80). An elevated RI (≥ 0.70) indicated impaired renal function and poor outcomes. Absent diastole (RI = 1.0) was an ominous sign, with 7% of patients requiring dialysis or transplantation.

    Conclusions:

    • Resistive index (RI) is typically elevated in established diabetic nephropathy but often normal in early clinical stages.
    • Renal Doppler US provides an accurate assessment of renal function in DKD.
    • Doppler US does not offer a significant advantage over conventional tests for evaluating diabetic renal disease.
