Background:

  • Hexagonal keratotomy (HK) is a refractive surgical procedure.
  • This study investigates the complications associated with HK.

Observation:

  • A review of 15 eyes from 10 patients undergoing HK revealed significant complications.
  • Common issues included glare, photophobia, fluctuating vision, and irregular astigmatism.
  • Severe complications such as corneal edema, perforation, bacterial keratitis, cataract, and endophthalmitis were observed.

Findings:

  • Wound healing abnormalities and anterior corneal displacement were frequent findings.
  • Eight eyes experienced a loss of best-corrected visual acuity by two or more Snellen lines.
  • Histologic analysis showed variable wound depth and gaping, indicating poor wound healing.
