Persistent postoperative urinary retention (PU) can necessitate secondary prostate surgery, particularly in elderly men. Pre-existing voiding disorders should be identified and treated before initial surgery to prevent this complication.

Area of Science:

  • Urology
  • Surgical Outcomes

Context:

  • Postoperative urinary retention (PU) is a known complication following surgical procedures.
  • Persistent PU may lead to unplanned, secondary prostate surgery.
  • The incidence of secondary prostate surgery due to PU after other operations is not well-established.

Purpose:

  • To determine the frequency of secondary prostate surgery required due to persistent postoperative urinary retention.
  • To identify risk factors and patient populations susceptible to this complication.

Summary:

  • A retrospective study analyzed 258 prostate surgeries over two years.
  • Twenty cases (7.8%) were secondary to persistent PU after other surgeries, primarily herniotomies and total hip arthroplasties.
  • In men over 70, secondary prostate surgery occurred in 4% after herniotomy and 11% after hip arthroplasty.

Related Experiment Videos

  • Eighty-five percent of patients undergoing secondary prostate surgery had pre-existing prostatism symptoms.
  • Impact:

    • Highlights the need to screen elderly men for voiding disorders before elective surgery.
    • Suggests that treating pre-existing lower urinary tract symptoms may prevent secondary prostate procedures.
    • Informs surgical planning and patient selection to mitigate risks of PU and subsequent interventions.
