Neoadjuvant hormonal therapy in prostate cancer - impact of PSA level before radiotherapy.

Background:

  • Acute lymphoblastic leukemia (ALL) is a high-grade malignancy often requiring intensive treatment.
  • Remission in ALL does not always preclude localized relapse.
  • Ocular involvement in pediatric ALL can manifest in various ways.

Observation:

  • A 3.5-year-old boy with diagnosed high-grade ALL presented with leukemic infiltration and hypopyon despite clinical and hematological remission.
  • Diagnosis was confirmed through clinical eye examination and ultrasound.
  • Histological examination revealed leukemic infiltration in the chorioidea, iris, ciliary body, nuchal region, spleen, and testicles.

Findings:

  • The patient exhibited leukemic infiltration of the iris with hypopyon.
  • Histopathology confirmed widespread leukemic infiltration beyond the ocular structures.
  • The ocular finding occurred during a period of systemic ALL remission.

Implications:

  • Isolated leukemic infiltration of the iris can be an early indicator of ALL relapse.
  • Prompt initiation of appropriate therapy is crucial upon detection of such signs.
  • Ophthalmological surveillance may play a role in early relapse detection in pediatric ALL patients.
