Background:

  • Primary hepatocellular carcinoma (HCC) is a significant global health concern.
  • Elevated levels of coagulation factors are occasionally observed in HCC patients.
  • Antithrombin III (AT-III) is a key regulator of coagulation, typically decreasing in active disease.

Observation:

  • A 53-year-old male presented with HCC, negative for viral markers (HBV, HCV).
  • Imaging revealed multiple nodules in the right hepatic lobe; PIVKA-II was elevated, AFP was normal.
  • Coagulation tests showed significantly increased antigenic quantity and activity of Antithrombin III (AT-III).

Findings:

  • Immunohistochemical analysis confirmed HCC (Edmondson I) with positive staining for AT-III.
  • The tumor cells themselves were identified as the source of the abnormally high AT-III levels.
