Posttraumatic osteolysis (PO) of the os pubis is a rare condition in postmenopausal women, often linked to osteopenia and insufficiency fractures. Sacral fractures commonly accompany PO, but typically heal, unlike the pubic condition.

Area of Science:

  • Orthopedics
  • Radiology
  • Pathology

Background:

  • Posttraumatic osteolysis (PO) of the os pubis presents as rapidly progressive destructive changes on radiographs.
  • This entity is often observed in postmenopausal women, frequently with underlying osteopenia.

Purpose of the Study:

  • To describe the clinical and radiographic features of posttraumatic osteolysis of the os pubis.
  • To highlight the association with sacral insufficiency fractures and discuss diagnostic implications.

Main Methods:

  • Retrospective analysis of 14 patients diagnosed with posttraumatic osteolysis of the os pubis.
  • Radiographic, CT, scintigraphy, and MR imaging were utilized to evaluate the pubic and sacral conditions.

Main Results:

Related Experiment Videos

  • All 14 patients were postmenopausal women; 7 had osteopenia and 7 had insufficiency fractures as the primary event.
  • Four patients exhibited bilateral symmetrical PO. All 14 patients had concomitant sacral insufficiency fractures.
  • While PO did not show bony healing, sacral fractures healed clinically and radiographically in 8 followed patients.

Conclusions:

  • Posttraumatic osteolysis of the os pubis is a distinct entity, often associated with sacral insufficiency fractures due to pelvic instability.
  • Awareness is crucial to prevent misdiagnosis as metastatic lesions and avoid unnecessary extensive diagnostic workups.
